Abstract
The NATO Performance Assessment Questionnaire (PAQ) was developed to obtain a large database on fatigue, motion sickness and task performance problems associated with ship motions in the naval environment. The PAQ was deployed on seven ships of the NATO Standing Naval Forces Atlantic fleet (STANAVFORLANT), during combat training exercises in February and March of 1997. Overall, 1026 personnel returned PAQ booklets completed over an 11 day period, providing a very large database on fatigue and motion sickness symptomatology, and on coincidental task performance problems. Additionally, two ships were instrumented to measure motions. This technical memorandum documents the PAQ physicla format and content, including all five languages used in the NATO deployment. Response rates from each ship are defined, and personnel demographics are summarized. The entry of data from PAQ booklets into a computer database is described, and a reliability assessment of responses from each ship is presented.
