- Defence R&D Canada – Toronto contracted Wendell Hull & Associates, Inc. (WHA) to determine the most probable cause and origin of a fire that occurred in a semi-closed circuit underwater breathing system called the Canadian Underwater Mine-countermeasures Apparatus, Version 2 (CUMA V2). WHA reviewed the incident details, system components, materials of construction, and operational procedures and conditions. They inspected the burned hardware and disassembled individual components as required to characterize the burn and melt-flow patterns. Key materials were sampled as required and chemical analyses on samples were done to obtain positive material identification. The most probable origin of the fire was determined to be the non-metallic, high-pressure seat in the Oxygen Regulator. The evidence indicated that operationally induced ignition mechanisms and incompatible materials were causative factors in the ignition and propagation of the fire. WHA made recommendations to reduce the likelihood of future fires and to increase the fault tolerance of the system if another fire does occur.
