FAILURE-MODES-AND-EFFECTS ANALYSIS OF THE DCIEM HYGE FACILITY FOR MAN-RATED EXPERIMENTS

Abstract
In order to upgrade the HyGe Crash Simulator at DCIEM's Impact Studies Facility for man-rated test programs that utilize human test subjects on the impact sled, a thorough review of the electric, pneumatic, hydraulic, and mechanical systems is documented. For background information, a brief description of the routine operation of the Crash Simulator in unmanned tests is given. For the analysis of manned test programs, the various possible modes of failure of all significant components is then ascertained and tabulated, and the effects of each failure on the safety of a sled firing is determined; in particular, as it relates to the HyGe staff personnel and test subjects.
